WILMINGTON — ChristianaCare’s Center for Hope and Healing at Wilmington Hospital has opened a second location at the Riverside Medical Arts Complex at 700 W. Lea Blvd. in Wilmington. The new location incorporates a primary care practice for this patient population.
The new practice, named ChristianaCare Primary Care – Center for Hope and Healing at Riverside, provides a full range of primary care services as well as care for significant behavioral health conditions or serious social needs.
Patients who receive care at the Center for Hope and Healing struggle with conditions such as bipolar disorder or schizophrenia in addition to chronic conditions such as diabetes or hypertension. Also, the patients may be challenged with social health needs, including homelessness or food insecurity.
“By addressing both medical and non-medical needs that can impede good health, the Center’s innovative level of extended care will help patients achieve better health and prevent unnecessary emergency department visits, hospitalizations and readmissions,” Anthony Tramontozzi, D.O., primary care physician at the new practice, said in a prepared statement.
