DOVER — BAYADA Home Health Care at Bayhealth recently celebrated five years providing in-home health care, including nursing and therapy services, to residents of central and southern Delaware.
A special anniversary theme, “The Impact of Partnership,” was introduced to mark the occasion and spotlight achievements of the joint venture, created between leading nonprofit home health care provider BAYADA Home Health Care and Bayhealth. The collaboration was designed to offer a seamless transition for clients from the hospital into their homes, where they can receive high-quality, short-term care while recovering after an illness, injury or surgery, or where they can help manage a chronic condition.
“When patients are discharged from our hospitals, we’re committed to coordinating our patients’ next phase of care,” said Bayhealth President and Chief Executive Officer Terry M. Murphy, FACHE. “As we celebrate five years of successful partnership as BAYADA Home Health Care at Bayhealth, we’re also celebrating the lives impacted and enhanced. Many of our patients can recover in the comfort of their own homes with the help of skilled and compassionate clinicians.”
Five achievements to mark five years:
Since launching in 2017, BAYADA Home Health Care at Bayhealth has hit major milestones to indicate its impact on the local community. It has:
2. Served over 800 Bayhealth Inpatient Rehab clients discharged to home
3. Achieved 75th percentile in Delaware state outcome roll-up scores
4. Created specialized heart failure and pulmonary clinical teams to expand clinical excellence in homes
5. Provided uninterrupted care during the pandemic, including to those with highly complex needs
