Form available for Delawareans having trouble with DelVAX

DOVER — Delawareans experiencing any trouble accessing their immunization records via the new DelVAX portal should complete a public portal data form, according to the Delaware Division of Public Health.

The new site can assist those who need to provide proof of COVID-19 vaccination but have lost their vaccination card. While DPH does not replace the card, the vaccination record is displayed through the page.

The portal is available here.

Visitors must enter the last name, first name, date of birth, gender and relationship to the person whose record is being sought. They then must choose to receive a code via text or email to access the immunizations.

When the code is sent, visitors must enter it on the “Verification Code” screen and finally click “Verify.”

If a match is not made from entered information, a link to the public portal data form becomes available. Instructions on how to submit the form are also found there.

“It should be filled out and sent to the program, so we can update the information,” said DPH spokeswoman Mary Fenimore. “It should take between one to three business days. ... We’ll update the record and send an e-mail when it is complete.”

Ms. Fenimore said the system will allow two attempts before a visitor is locked out of attempting to access the portal for 30 minutes.

Delawareans can contact the program at 800-282-8672 or delvax@delaware.gov for assistance. Ms. Fenimore added that staff are handling the phones and managing the DelVAX mailbox, with support from Delaware National Guard personnel.

Responses to emails and calls received after regular business hours or on weekends may take up to 48 hours.
